Job summary

Qualifications

• Bachelor’s degree in Business Administration, Social Work, Marketing or related field; plus relevant experience.
• Associate’s degree with three years experience required.

Responsibilities

• Coordinate pre-admission and admission processes with patients, families, and referrals.
• Verify patient room readiness prior to admission.
• Support patients and families through the transition and admission process.
• Conduct regular patient visits to ensure a smooth admissions experience.
• Orient patients to the center environment and communicate effectively with teams.
• Participate in Interdisciplinary Team discussions to assist navigation of care.
• Maintain contact with central intake and coordinate back-up plans for admissions.
• Contribute to development of outreach plans and partnerships with healthcare partners.
