Admissions Coordinator Responsibilities

• Coordinate the admission process for new residents, including gathering and reviewing necessary documentation, conducting tours, and introducing residents to our care team
• Work closely with families and healthcare professionals to gather medical and social history information to create a comprehensive patient profile
• Collaborate with our health care teams to ensure accurate and timely communication of resident information and needs
• Maintain accurate and up-to-date records of resident admissions, transfers, and discharges
• Develop and maintain relationships with healthcare professionals, social workers, and other referral sources to ensure a steady flow of referrals
• Provide exceptional customer service to residents, families, and healthcare professionals

Admissions Coordinator Requirements

• Prior of experience in healthcare administration or a related field
• Strong communication and interpersonal skills
