Job summary

OakBend Medical Center is seeking an Intake Coordinator to manage the referral and admission process for patients considering placement in a skilled nursing facility. The role acts as the primary liaison among referral sources, hospital clinical teams, patients and families, and insurers.

You will ensure timely processing of referrals, review of medical and financial information, and coordination of insurance authorizations, while maintaining confidentiality in a fast-paced healthcare

Qualifications

• High school diploma or equivalent; associate or bachelor’s degree preferred.
• Two or more years in healthcare admissions or related field preferred.
• Experience with SNF admissions or hospital discharge processes strongly preferred.
• Strong knowledge of referral, admission, and discharge processes; excellent communication.

Responsibilities

• Coordinate and facilitate the referral and admission process for SNF placements.
• Serve as liaison between referral sources, hospital clinical teams, patients and families, insurance providers, and partners.
• Ensure referrals are processed accurately and efficiently; obtain and review clinical and financial information.
• Address insurance authorization requirements and communicate throughout the admission process.
• Maintain confidentiality and handle multiple referrals in a fast-paced environment.
