Job summary

PAM Health is seeking a Patient Assessment Coordinator to manage data collection for Medicare reporting and ensure CMS compliance. You will oversee IRF/PAI forms, coordinate documentation, and educate staff across the facility.

Applicants should have licensure (RN/CRRN preferred), several years in acute hospital, rehabilitation, or LTACH settings, and experience with the eRehabData system. We offer comprehensive benefits and a 401(k) match.

Qualifications

• Licensed clinician (RN preferred) with current state licensure.
• ASN required; Bachelor’s degree preferred for nursing.
• Master’s degree required for OT/PT/SLP.

Responsibilities

• Coordinate data collection for IRF/PAI and CMS reporting.
• Ensure supporting documentation is complete and timely.
• Provide staff education to ensure CMS compliance.
• Monitor IRF/PAI scoring accuracy and input data into eRehabData.
• Coordinate patient information from medical records and complete IRF/PAI sections.
