Job summary

PAM Health in Texas seeks a Patient Assessment Coordinator to coordinate data collection for the IRF/PAI and CMS quality reporting. This role ensures accuracy, timely documentation, and staff education to maintain compliance with CMS guidelines within the eRehabData system.

The coordinator reviews IRF/PAI forms, sets completion dates with the multi-disciplinary team, and interfaces with patient records to capture care needs and functional status.

Qualifications

• ASN required; Bachelor’s degree preferred for nursing. Master’s degree required for OT/PT/SLP.
• Current Basic Life Support (BLS)/CPR card.
• A minimum of three to five years of experience in a patient assessment coordinator role or equivalent in acute hospital, rehabilitation, or LTACH setting.

Responsibilities

• Coordinate the process of providing accurate data to CMS entities involved in Medicare patient data reporting.
• Oversee data collection on the IRF/PAI and ensure supporting documentation is present.
• Provide staff education to ensure CMS guidelines compliance and accurate scoring of functional status.
• Set proscribed IRF/PAI completion dates for the multi-disciplinary team and coordinate with patient records.

Education

ASN (required); Bachelor’s degree preferred for nursing; Master’s degree required for OT/PT/SLP

Graduate of accredited OT/PT/SLP or RN program with current licensure
