Job summary

PAM Health Specialty and Rehabilitation Hospital of Covington is seeking a Patient Assessment Coordinator to coordinate data collection for Medicare-related reporting, ensure CMS compliance, and support IRF/PAI processes using the eRehabData system.

The role requires 3–5 years in patient assessment coordination or related hospital setting, state licensure as OT/PT/SLP/RN preferred, and a commitment to accuracy, education for staff, and timely completion of IRF/PAI forms.

Qualifications

• Licenses/Certifications: Graduate of an accredited OT, PT, SLP, or RN program with current licensure in the state. CRRN preferred.
• Experience: 3–5 years in a patient assessment coordinator role in acute hospital, rehabilitation, or LTACH setting.

Responsibilities

• Coordinate data collection for the IRF/PAI and CMS reporting.
• Review IRF/PAI forms for accuracy and timeliness, ensuring supporting documentation.
• Input IRF/PAI information into the eRehabData system and support staff training on CMS guidelines.

Education

ASN required; Bachelor’s degree preferred for nursing. Master’s degree required for OT/PT/SLP.
