Job summary

PAM Health Rehabilitation Hospital of Houston Heights is seeking a Patient Assessment Coordinator to oversee IRF/PAI data collection and ensure accurate CMS compliant reporting. You will work with the multi-disciplinary team to set completion dates and maintain documentation accuracy.

The role requires a licensed professional (OT/PT/SLP or RN) with an ASN and 3–5 years of relevant experience in acute hospital, rehab, or LTACH settings. Familiarity with the eRehabData system is preferred.

Qualifications

• Requires degree/licensure in OT/PT/SLP or RN with state licensure.
• 3–5 years in a patient assessment coordinator role or related rehab setting.

Responsibilities

• Coordinate data collection for IRF/PAI forms and ensure timely completion.
• Provide staff education to ensure CMS guideline compliance and accurate scoring of functional status.
