Job summary

PAM Health Rehabilitation Hospital of Allen is seeking a Patient Assessment Coordinator to coordinate IRF/PAI data collection, ensure documentation accuracy, and support CMS reporting requirements.

Responsibilities include working with the multi-disciplinary team, ensuring timely form completion, and providing staff education on CMS guidelines and FIM scoring. A clinical background in OT/PT/SLP/RN is preferred with appropriate licensure.

Qualifications

• Graduate of an accredited OT, PT, SLP, or RN program with current state licensure.
• ASN required; Bachelor’s degree preferred for nursing; Master’s degree required for OT/PT/SLP; BLS/CPR card.
• Three to five years of experience as a patient assessment coordinator or similar role.

Responsibilities

• Coordinate the IRF/PAI data collection with the multidisciplinary team and ensure timely completion.
• Verify supporting documentation and input accurate information into the eRehabData system.
• Educate staff on CMS guidelines and monitor scoring for functional status and care needs.
