Job summary

PAM Health Specialty and Rehabilitation Hospital of Covington is seeking a Patient Assessment Coordinator to oversee data collection for IRF/PAI under CMS guidelines. You will ensure timely, accurate documentation and educate staff in the eRehabData system.

The role requires 3–5 years in a patient assessment setting within acute hospital/rehab LTACH environments, with appropriate licensure and strong attention to detail.

Qualifications

• Graduate of an OT, PT, SLP, or RN program with州 licensure in the state (CRRN preferred).
• 3–5 years of experience in a patient assessment/coordinator role in acute hospital/rehab LTACH settings.

Responsibilities

• Coordinate data collection for IRF/PAI forms, ensuring accuracy and timeliness.
• Review IRF/PAI forms completely and ensure CMS guidelines compliance.
• Input information for IRF/PAI into eRehabData; educate staff on required processes.
• Set completion dates for IRF/PAI by multi-disciplinary team; develop policies and procedures.
