Job summary

UCLA Health is seeking a Patient Service Representative for the Ventura Primary Care Clinic. You will support physicians and patients, act as a liaison, and help patients access services across the health system.

You will coordinate with referring physicians, guide patients to resources, and conduct follow-up using telephone and EPIC/Care Connect. This role emphasizes service excellence within a collaborative care environment.

• Ability to provide coverage for Community Clinics as assigned.
