Job summary

Mosaic Life Care seeks an Access Representative to support patient access and continuity of care at multiple facilities. You will act as a receptionist, manage appointments, and assist with data entry and documentation to ensure efficient patient flow.

Strong attention to detail, excellent communication, and the ability to handle multiple tasks in a fast-paced environment are essential. On-site work with travel between system facilities may be required.

Qualifications

• Proficiency in data entry and accurate patient information handling.
• Strong customer service and communication with patients and staff.
• Knowledge of medical terminology is preferred.

Responsibilities

• Schedule, reschedule, and cancel patient appointments with clinicians and front desk staff.
• Enter patient data into systems accurately (registration, records, visits).
• Communicate with patients about appointments, forms, and documentation.
• Process release of information and consent forms per regulations.
