Think Healthcare in Omaha, NE is seeking an administrative professional to triage patients, contact them for information, coordinate referrals, and communicate with the primary care team to ensure tasks are completed.

You will register new patients, obtain records, coordinate referrals, identify care gaps, and ensure all documents are saved correctly in the EHR system, while maintaining confidentiality and professional demeanor.

Qualifications

• High School diploma or GED required.
• Minimum 1 year of experience in the medical field utilizing EHR.
• Additional 1 year of experience in a clinical setting preferred (Family Practice/Internal Medicine).

Responsabilités

• Coordinate registration of new patients, collecting demographics and records.
• Coordinate patient referrals to specialists and collect referral records.
• Identify and reconcile gaps in care and ensure documentation in EHR.
• Maintain proper saving of documents in patients' electronic health records.
