Responsibilities

• Receive intake information and enter in the computer.
• Complete intake forms and route them for admission approval.
• Contact patients to verify contact information and physician orders.
• Coordinate with business development, discharge planners, and facility contacts to admit patients promptly.
• Comply with HIPAA and the agency's Compliance Program.
• Maintain case management notes and confidentiality of patient records.
• Provide effective communication to clients, families, team members, and health professionals.
• Participate in in-service programs and maintain/upgrade competency.
