Job summary

Vitality Hospice seeks an Intake Coordinator to serve as the primary contact for referrals, coordinating the intake process from referral through admission. You will collaborate with hospitals, physician offices, SNFs, case managers, and the clinical team to ensure timely admissions and complete documentation.

The role requires excellent communication, organization, and attention to detail, with a compassionate approach to patients and families.

Qualifications

• High school diploma or GED required; Associate's or Bachelor's degree preferred.
• Previous experience in hospice, home health, healthcare intake, admissions, scheduling, medical office, or customer service preferred.
• Excellent communication, organizational, and multitasking skills.
• Strong attention to detail and ability to work in a fast-paced environment.
• Tech-savvy with the ability to quickly learn new software and healthcare technology platforms. Proficient in Microsoft Office and EMR systems.
• Knowledge of hospice or home health regulations is a plus.
• Compassionate, dependable, and committed to providing exceptional customer service.

Responsibilities

• Receive and process incoming referrals via phone, fax, email, and electronic referral platforms.
• Communicate with hospitals, physician offices, SNFs, case managers, and other referral sources to obtain required documentation.
• Collect, verify, and accurately enter patient demographics, insurance information, physician orders, and clinical documentation into the EMR system.
• Coordinate with the clinical and admissions team to ensure timely patient admissions and scheduling.
• Verify insurance eligibility and ensure all required documentation is complete prior to admission.
• Maintain accurate referral records and monitor the status of pending referrals.
• Communicate professionally and compassionately with patients, families, referral sources, and internal staff.
• Maintain confidentiality and comply with HIPAA and all company policies.
• Assist with administrative duties related to the intake process.
• Collaborate with clinical leadership to ensure an efficient, patient-focused admissions experience.
