Duly Health and Care in East Dundee, IL is hiring a Patient Access Conversion Specialist to help patients navigate scheduling and information needs. The role is onsite at a local medical office, with hours Monday through Friday from 8:00 a.m.

to 5:00 p.m., supporting a busy front desk and care coordination. You will verify patient information in EPIC, schedule appointments, and maintain a positive patient experience while handling high call volumes.

• Answers heavy call volumes to fulfill patient needs over the phone.
• Patient appointment scheduling.
• Sets the tone for a positive experience.
• Verifies patient information, while documenting in EPIC.
• Performs all other duties / responsibilities as assigned.
• Reviewing schedule.
• Pre-screen patients based on departmental protocol.
• Monitor schedules daily, ensuring all open slots are filled on the same day.
• Daily monitoring of patient scheduling reports.
• Review the Daily Appointment Report and prepare charts as appropriate.

JOB DESCRIPTION

Patient Access Conversion Specialist

Hours: Monday through Friday between 8a - 5pm

Onsite at Local medical Office- East Dundee

Responsibilities:

Qualifications:

• High school diploma or GED
• Three years of patient/client-facing experience

The compensation for this role includes a base pay range of $16.34 - $24.52 , with the actual pay determined by factors such as skills, experience, education, certifications, geographic location, and internal equity. Additional compensation may be available through shift differentials, bonuses, and other incentives. Base pay is only a portion of the total rewards package.
