Job summary

Responsibilities

• Assist patients in understanding their diagnosis, treatment options, and the healthcare process.
• Coordinate appointments, follow-ups, and referrals to ensure timely access to healthcare services.
• Provide emotional support and education to patients and their families, helping them make informed decisions about their care.
• Identify and address barriers to healthcare access, such as transportation, financial constraints, and language differences.
• Collaborate with healthcare providers, social workers, and community resources to develop comprehensive care plans for patients.
• Maintain accurate and confidential patient records, documenting interactions and progress.
• Advocate for patients' needs within the healthcare system to ensure they receive optimal care.
• Conduct outreach activities to educate the community about available healthcare services and resources.
