Job summary

ScionHealth is seeking an experienced Patient Access & Intake Coordinator to facilitate smooth patient admissions and manage referrals. You will work with the Business Development and Central Access and Authorizations Team (CAAT) to ensure accurate data, timely approvals, and coordinated bed and service needs.

Your role requires strong communication, attention to detail, and proficiency with Meditech and referral platforms.

Qualifications

• High School diploma or GED required.
• Associates or Bachelors preferred in healthcare or related field.
• Healthcare experience preferred; post-acute care experience a plus.
• Fluent English required.

Responsibilities

• Launch referral management by creating patient accounts in referral platforms and Meditech.
• Validate data, gather documentation, and set up initial eligibility and authorizations.
• Coordinate bed assignments, admissions paperwork, and communication with facility leadership.
• Maintain accuracy of the pending referral log and related systems.
• Participate in ongoing education and process improvements as part of CAAT.
