Job summary

The Salvation Army Ferguson Community Empowerment Center is seeking a Pathway to Health Case Manager to recruit, enroll, and coordinate interventions for participants across Pathway of Hope and Pathway to Health programs. The role is community-based with frequent local travel and requires a strong commitment to client service.

This full-time position involves outreach, coordination with partner agencies, data entry in SIMS & HMIS, and timely reporting.

Qualifications

• Bachelor’s degree in human services field (BSW/MSW preferred).
• At least 1 year related work experience.
• Outgoing personality with good communication skills.
• Ability to work with diverse cultures; strong organizational skills.
• Knowledge of Salvation Army resources is helpful.
• Valid driver’s license; travel 70% community-based; 30% office-based.

Responsibilities

• Recruit and enroll participants for Pathway programs (Pathway of Hope and Pathway to Health).
• Build relationships with outside agencies for referrals.
• Conduct pre-screening and intake using specified assessment tools.
• Set goals with clients and formulate action plans; conduct follow-up meetings.
• Record client information in SIMS & HMIS within 48 hours and provide reports.
• Distribute resources and coordinate referrals; liaise with agencies as needed.
• Maintain confidentiality and comply with Safe From Harm guidelines.
• Represent The Salvation Army in service activities and events.
