Job summary

The Salvation Army Blue Valley Corps is seeking a Case Manager for the Pathway of Hope program. You will coordinate social services, assess families, provide direct assistance and referrals to help break cycles of crisis and enhance housing stability and financial resources.

Responsibilities include outreach, intake, eligibility screening, case planning, home visits, and collaboration with staff and community partners.

Qualifications

• Degree in human services or related field; experience may substitute for degree.
• Minimum two years of case management in a social service program.
• Ability to engage diverse populations with sensitivity and cultural awareness.

Responsibilities

• Coordinate POH services using strength-based case management.
• Conduct intake, eligibility screening, and referrals.
• Develop and monitor individualized case plans for housing stability and income growth.
• Conduct home visits and schedule regular progress meetings.
• Maintain confidential case files and ensure quality data entry in SIMS.
