Job summary

The Salvation Army Ferguson Community Empowerment Center in Ferguson, MO seeks a full-time PoH/ESS Case Manager to provide intake, assessment and services for ESS assistance and Pathway of Hope (PoH) clients. Responsibilities include goal setting, service coordination, and budgeted assistance for basic needs.

Minimum qualifications include a Bachelor’s degree in human services with related experience, strong communication and leadership skills, and ability to work with diverse populations.

Qualifications

• Bachelor’s degree in human services with 1 year related work experience.
• Good communication skills and ability to work with diverse cultures.
• Reliable transportation with valid Driver’s License and pass TSA MVR check.

Responsibilities

• Deliver the Pathway of Hope service including intake, assessment and goal setting.
• Conduct follow-up case management meetings and track progress on action plans.
• Input client information into SIMS and maintain confidentiality.
• Provide on-site and outreach assistance for emergency services and food distribution.
• Coordinate with community resources and other Corps programming.
