Job summary

You will collaborate with community programs, maintain client records, and provide on-site and outreach support to those seeking emergency assistance. Eligibility screenings and resource referrals are essential components of the role.

Qualifications

Bachelor’s degree in human services with 1 year related experience.
Must have an outgoing personality with good communication skills.
Strong leadership and organizational abilities required.

Responsibilities

Provide intake and assessment for ESS clients and manage Pathway of Hope services.
Coordinate with community agencies and referral resources.
Network to maximize services and track client goals in SIMS.
Distribute resources and assist clients with eligibility and referrals.
Maintain current community resource guides and outreach as needed.
