Job summary

MBK Senior Living in Gilbert, AZ is seeking a Health Services Coordinator to support the health services team with administrative and clinical tasks. The role preserves resident dignity, coordinates care plans, and ensures policy compliance.

This weekend, part-time position requires an active RN, LVN, or LPN license, 2+ years in healthcare, strong communication, and MS Office proficiency. Be part of a team focused on quality of life and safety.

Qualifications

• Must complete required background clearances, health screening and provide negative TB test results within 7 days of employment.
• Valid state driver’s license and reliable transportation.
• Excellent communication skills including the ability to speak, write and read English.

Responsibilities

• Coordinate assessments and evaluations of potential residents and make admission recommendations.
• Draft initial individualized service plans and update as needed.
• Review service plans with responsible parties and Executive Director as requested by DHS.
• Ensure continuity of the assisted living residents’ total care regimen.
• Provide training on medication delivery, safety protocols, and state requirements.
• Maintain accurate documentation and perform required duties timely.
