Qualifications

• Active RN/LVN/LPN license in good standing with CEU requirements and renewals.
• Ability to operate MS Office and basic computer skills for documentation and scheduling.

Responsibilities

• Coordinate assessments and admission recommendations in line with policy and regulations.
• Draft and update Individualized Service Plans (ISP) with DHS oversight.
• Ensure continuity of resident care and assist with medication delivery and safety procedures.
• Coordinate prescription orders with doctors and manage pharmacy delivery and records.
• Support the DHS with training and education on essential topics and documentation.
