Qualifications include an active RN/LVN/LPN license and at least 2 years of related experience, with strong organizational and communication skills.

Qualifications

• Must have RN/LVN/LPN license in good standing.
• Minimum 2+ years of healthcare experience.
• Strong communication, organization, and computer skills.

Responsibilities

• Conduct and coordinate resident assessments and admission recommendations.
• Draft and update ISP/service plans in collaboration with DHS.
• Coordinate medication delivery and pharmacy communications.
• Provide training on safety, documentation, and state requirements.
• Perform basic nursing functions and monitor vital signs.
• Ensure compliance with regulations and MBK policies.
