Qualifications

• Must hold an active RN, LVN or LPN license.
• Minimum 2+ years experience in a healthcare setting.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct and coordinate resident assessments and admission recommendations.
• Draft and update individual Service Plans.
• Coordinate medication delivery and pharmacy communications.
• Provide training on essential clinical topics per DHS direction.
• Cover department duties during absences and act as medication technician if needed.
• Maintain accurate records and ensure regulatory compliance.
