Job summary

MBK Senior Living in Utah seeks a Health Services Coordinator (LVN/LPN) to support the health services team with administrative and clinical duties under the Director of Health Services. You will ensure residents receive appropriate care, maintain service plans, and coordinate pharmacy and doctor communications.

The role requires active LVN/LPN license, 2+ years of relevant experience, strong communication, organization, and computer skills.

Qualifications

• Active RN/LVN/LPN license in good standing.
• 2+ years of related healthcare experience.
• Must pass background clearances and health screenings including TB test results.
• Excellent communication, organizational, and time management skills.

Responsibilities

• Conduct and coordinate resident assessments and admission recommendations.
• Draft and update Individualized Service Plans (ISP).
• Coordinate prescription orders with doctors and pharmacies.
• Provide medication administration and basic nursing tasks as directed by DHS.
• Train and educate staff on essential topics and resident care policies.
• Ensure safety and quality standards in daily operations.
