Job summary

Community Health Resources (CHR) in Coventry, CT, is seeking a part-time Case Manager for Hendee Road House. This weekend program requires Saturday and Sunday 7am-3pm, 16 hours per week, delivering patient-centered case management within a residential setting.

You will conduct assessments, connect clients to services, collaborate with treatment teams, provide crisis intervention, and document all interactions in line with agency standards.

Qualifications

• Bachelor’s degree in human services or equivalent experience.
• Minimum 2 years of mental health experience.
• Valid driver's license.

Responsibilities

• Uses therapeutic models to aid clients in understanding and managing symptoms.
• Conducts program-specific assessments and screens.
• Provides case management in collaboration with treatment teams.
• Demonstrates competency with population-specific needs and skills.
• Teaches and supports emotion regulation and daily living skills.
• Documents all services and contacts per agency standards.
• Transports clients as required.
• Participates in staff meetings and trainings.
