Job summary

(CHR) Community Health Resources, Inc. is seeking a part-time Case Manager for Strickland, Manchester, CT. This role involves supporting an intensive residential program with 24-hour services and varied shifts (1st and 2nd).

The Case Manager will use evidence-based approaches, maintain collateral contacts, and document services in the EMR to ensure client-centered care. A bachelor’s in human services and driver’s license are required; two years in mental health preferred.

Qualifications

• Bachelor’s degree in human services required.
• At least two years of mental health experience preferred.
• Valid driver’s license.

Responsibilities

• Utilizes therapeutic models to help clients understand and manage symptoms.
• Maintains family and collateral contacts to ensure client-centered services.
• Assesses and involves natural supports in treatment planning and discharge.
• Provides case management, clinical and crisis intervention supports as indicated.
• Uses EMR to document all services and contacts.
