CHR Career Opportunities is seeking a Case Manager for the Eastern Region Service Center in Willimantic, CT. This full-time role (40 hours/week) follows a Monday–Friday schedule and involves direct care case management for clients with mental health and substance use needs.

Qualifications

• Bachelor’s degree in human services preferred, or related education with equivalent work experience.
• Valid driver’s license.

Responsibilities

• Utilizes therapeutic models to aid clients in understanding and managing their symptoms.
• Provides case management and coordinates with client’s treatment teams.
• Documents all services and contacts in compliance with agency standards.
• Transports clients as required by program.
• Participates in staff meetings and trainings.
