Job summary

Community Health Resources, Inc. is seeking a Part-time Case Manager to support residents at Hendee Road House in Coventry, CT.

The role requires 16 hours weekly, with Saturday and Sunday shifts from 7am to 3pm, delivering case management and crisis intervention while collaborating with treatment teams. Responsibilities include documenting services, transporting clients, and participating in regular meetings.

Qualifications

  • Bachelor’s degree in human services preferred, or education plus equivalent work experience.
  • At least two years’ experience in a mental health setting preferred.
  • Valid driver’s license.

Responsibilities

  • Utilizes therapeutic models to aid clients in understanding and managing symptoms in various environments.
  • Conducts program-specific assessments and screens as required by the program.
  • Provides case management and connects clients to pro-social activities; offers crisis intervention and de-escalation as needed.
  • Demonstrates age- and population-appropriate competencies and plans activities to increase skills and independence.
  • Documents all services and contacts in line with agency standards.
  • Transports clients as required by the program.
  • Participates in staff meetings, supervision, training, and other assigned meetings.
  • Performs additional duties as required.

JOB TITLE: Case Manager

EMPLOYMENT TYPE: Part-time, 16 hours/wk

SCHEDULED HOURS: Saturday and Sunday 7am-3pm

PROGRAM/LOCATION: Hendee Road House, Coventry, CT

ABOUT THE PROGRAM: Hendee Road House

Hendee Road Home is a 6 bed residential program for men with severe mental illness and are discharging from long term hospitalization. The program will address the full range of daily living needs of each individual with a focus on increasing independence and self-efficacy of individuals through 24-hour/7-day a week staff support in the residence and the community. Staff interventions focus on teaching skills around daily living, symptom management, coping skills, medication management, health & nutrition, healthy relationships, personal safety, money management, substance use education, linking to recovery supports, and providing sober activities. Staff will also provide crisis intervention in attempt to reduce need for hospitalizations and emergency room visits.

ABOUT THE POSITION: Case Manager
Duties & Responsibilities
  • Utilizes appropriate therapeutic models to aid clients in understanding and managing their symptoms in a variety of environments.
  • Conducts program specific assessments and screens as required by program
  • Provides case management. Works in close collaboration with the client’s natural supports and treatment teams. Connects to prosocial activities. Provides crisis intervention and de-escalation as needed.
  • Demonstrates competency in age related or specialty issues and developmental needs for each population served. Demonstrates program specific target population competencies.
  • Demonstrates the ability to teach and support the learning of a broad range of skills including those related to emotion regulation, age-appropriate activities and competencies and living in a home-like setting. Plan and actively participate in activities to support and increase skills and competencies.
  • Documents all services and contacts with clients, collateral contacts and appointments in compliance with agency standards.
  • Transports clients as required by program.
  • Participates in staff meetings, supervision, training and other meetings as assigned.
  • Additional duties as required.
QUALIFICATIONS

Education: Bachelor’s degree in human services preferred, or education plus equivalent work experience required.

Experience: At least two (2) years’ experience in a mental health setting preferred.

Licensure/Certification/Registration: Valid driver’s license.

COMPENSATION*

$19.2850/hour minimum, and up commensurate upon experience

*Actual rates are determined at the time of offer and are based on relevant experience and internal equity, as well as, adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
