Job summary

Mountain Ridge Center in New Hampshire is seeking a Part-Time Social Services Specialist to assess patients and families, identify social, emotional, and mental health needs, and connect them with appropriate services to maximize well-being and quality of life.

Work with the interdisciplinary team to develop care plans, conduct meetings with patients and families, assist with discharge planning, and coordinate community referrals in a compliant, patient-centered environment.

Qualifications

• Bachelor's degree in social work or human services field is required.
• State licensure/certification as required by state to practice in long-term care.
• Experience working in a Long Term Care facility.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life.
• Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.
• Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.
