PowerBack Rehabilitation in Camden, ME is seeking a Social Services Specialist to assess and address the social, emotional, and mental health needs of patients and residents in a nursing center, coordinating services to enhance quality of life. You will work with families and staff to support well-being and care outcomes.

The role involves collaborating with an interdisciplinary team, developing care plans, and guiding discharge planning while aligning with NASW ethics and applicable guidelines

Qualifications

• Bachelor's degree in social work or human services field is required.

Responsabilités

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being, including educating staff on the role of social services.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others to provide support and information to improve quality of life.
• Perform patient/resident discharge planning activities such as education, documentation and referrals to appropriate agencies and suppliers.
• Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.
