Job summary

PowerBack Rehabilitation in West Virginia seeks a Social Services Specialist to work with patients and families to identify social, emotional, and mental health needs and connect them with services to enhance well-being and quality of life.

The role requires collaboration with the interdisciplinary team to develop care plans, discharge planning, and coordination with community resources in compliance with NASW ethics and state guidelines.

Qualifications

• Bachelor's degree in social work or human services field required.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life.
• Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.
• Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.
