Job summary

West State Street is seeking a Social Services Specialist to enrich the lives of patients and residents by coordinating social, emotional, and mental health services within a long-term care setting.

You will collaborate with an interdisciplinary team to develop care plans, educate staff on social services, and support families. This role emphasizes discharge planning, community referrals, and improving overall quality of life for residents.

Qualifications

• Bachelor's degree in social work or human services field.
• Must possess any certifications/licensures as required by State of employment to practice in long-term care.

Responsibilities

• Collaborate with interdisciplinary team to promote and protect patient/resident rights and well-being; educate staff on the role of social services.
• Participate in developing an interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others to provide support and information to improve quality of life.
• Perform discharge planning: family/patient education, charting, and referrals to appropriate agencies/suppliers.
• Establish relationships and maintain contact and referral flow with community-based agencies/services for patient/resident benefit.
