Complete Care, Inc. is seeking a compassionate Social Worker to support residents and families in our long-term care setting. You will assess psychosocial and mental health needs and advocate for residents' rights within an interdisciplinary team.

The role emphasizes building rapport, independent and team collaboration, and ensuring residents receive personalized care. Must have LSW or CSW; long-term care experience preferred. Part-time, day shift; on-site in New Jersey.

Qualifications

• Must have LSW or CSW
• Previous long term care experience is preferred but not required
• Identify residents' psychosocial, mental and emotional needs
• Complete Social History and Psychosocial assessment of residents
• Work with interdisciplinary team to promote and protect residents' rights
• Prevent and address resident abuse as mandated by law and licensure
• Able to build and maintain rapport with residents and peers
• Work independently or as part of a group
• Computer knowledge required

Responsabilités
