PowerBack Rehabilitation in Lebanon, NH is seeking a Social Services Specialist (Social Worker) to assess psychosocial needs of residents and coordinate services to enhance well-being and quality of life. You will work with families and the care team to plan and implement comprehensive care plans.

Responsibilities include discharge planning, caregiver education, and building community referrals to support residents' social and emotional needs.

Qualifications

• Bachelor's degree in social work or human services field required.
• Must possess any certifications/licensures as required by State of employment to practice in long-term care.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life.
• Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.
• Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.
