Job summary

Mountain Ridge Center, part of Genesis Healthcare, is seeking a Part-Time Social Worker to support residents and their families by assessing social, emotional, and mental health needs and coordinating services to improve quality of life.

You will collaborate with the interdisciplinary team, participate in the plan of care, and facilitate meetings to provide information and support. Requirements include a BA in social work, state licensure as needed, and LTC experience.

Qualifications

• Bachelor's degree in social work or human services field.
• Must possess any certifications/licensures as required by State of employment to practice in long-term care.
• Experience working in a Long Term Care facility.

Responsibilities

• Enrich lives as a nursing center Social Services Specialist where you will work with patients/residents and their family members/significant others to identify strengths, social, emotional, and mental health needs and then obtain services to meet those needs.
• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life.
• Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.
• Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.
