Qualifications

• High school diploma or equivalent is required.

• Strong communication and organizational skills are required.
• Comfortable working with individuals from diverse backgrounds.
• Community Health Worker certification welcome but not required if other qualifications are met.

Responsibilities

• Complete social care screenings.
• Connect individuals with community resources and services.
• Make referrals and conduct follow-up.
• Verify eligibility and maintain accurate documentation.
• Communicate with clients, providers and community partners.
• Help people navigate barriers to needed support.
