Job summary

Harmonia Collaborative Care seeks a compassionate Social Care Navigator to help community members access health and well-being resources. Part-time, about 18–22 hours per week, with responsibilities including screenings, referrals, and follow-up.

Qualifications

• High school diploma or equivalent is required.
• 1–2 years of experience in care coordination, healthcare, community services, public health, nonprofit services or related field.
• Strong communication and organizational skills are required.
• Comfortable working with individuals from diverse backgrounds.
• Community Health Worker certification welcome but not required if other qualifications are met.
