Job summary

CareSource is seeking a Community Based Care Manager to collaborate with an interdisciplinary care team and community partners to improve the health and well-being of members through culturally competent care coordination.

You will facilitate ICT meetings, develop person-centered care plans, and coordinate with providers, facilities, and community resources to address behavioral, physical, and social determinants of health.

Qualifications

• Nursing degree from an accredited program or a health care bachelor’s degree is required.
• Current clinical license in RN, social work, or clinical counselor is required.
• Three years of nursing, social work, counseling, or health care experience is required.

Responsibilities

• Engage members using strength-based, trauma-informed assessments and health equity lens.
• Facilitate ICT meetings and coordinate care plans.
• Develop and update person-centered ICP with member preferences.
• Identify and address barriers to care and coordinate post-discharge planning.
