Job summary

Vineyard Post Acute is seeking licensed Speech-Language Pathologists to join our part-time therapy team in Petaluma, CA. The role focuses on assessing and treating residents with speech, language, voice, cognitive, and swallowing disorders, with collaboration across an interdisciplinary team.

Qualifications

• Valid California SLP license.
• SNF or geriatric rehab experience preferred; new grads welcome.

Responsibilities

• Assess and develop individualized treatment plans for residents with speech, language, voice, cognitive, and swallowing disorders
• Implement evidence-based therapy techniques to support communication and swallowing function
• Collaborate with an interdisciplinary team, including PTs, OTs, nursing staff, and physicians, to provide holistic care
• Educate residents and families on strategies for continued progress
• Maintain accurate documentation of evaluations, treatment plans, and progress reports
