Job summary

Sonoma Post Acute is seeking a part-time Speech-Language Pathologist for PRN coverage in our facility. The clinician evaluates residents, develops treatment plans, and coordinates with families, physicians, and the health team. Supervises CFY fellows and students as applicable.

The role requires state licensure as a Speech Therapist, CPR if required, and proficiency with Net Health and Point Click Care. Opportunities to educate staff and contribute to rehab program development are included.

Qualifications

• Bachelor’s degree in Speech-Language Pathology; license required.
• Master’s or Doctorate in Speech-Language Pathology preferred.
• Licensed as a Speech Therapist in the state.
• CPR certification, if required by licensure.
• Proficient with Net Health and Point Click Care systems.
• Continuing education requirements must be met.

Responsibilities

• Evaluate residents to determine speech therapy needs, functional limitations and rehab potential.
• Develop, implement, and update individualized treatment plans to improve function.
• Assist nursing with training of Restorative Aides as needed.
• Supervise CFY Speech Clinical Fellows and students per state rules.
• Communicate with physicians, nursing staff, residents and families about progress.
• Participate in care conferences and rehabilitation meetings as needed.
• Provide in-services on training programs for staff as needed.
• Complete timely, accurate documentation in accordance with policies.
