Vineyard Post Acute is seeking a dedicated Physical Therapist Assistant (PTA) to join our skilled rehab team in Petaluma, CA. We offer full-time and part-time roles with flexible scheduling to support work-life balance and quality patient care.

As part of the skilled nursing facility, you will assist licensed PTs, help residents regain mobility, and contribute to individualized plans while maintaining documentation and collaborating with nursing and therapy colleagues.

Qualifications

• Active California PTA license required.
• Strong clinical and communication skills.
• New graduates and experienced PTAs encouraged to apply.

Responsibilities

• Assist licensed PTs in implementing personalized treatment plans.
• Provide hands-on therapy to improve mobility, strength, and coordination.
• Educate residents and families on continuing exercises and rehabilitation techniques.
• Collaborate with nursing and therapy teams to ensure optimal patient outcomes.
• Maintain accurate documentation per state and facility guidelines.
