CCH - (Christian Church Homes) seeks a part-time Social Services Coordinator in Colorado Springs to help senior residents maintain independence and dignity. You will answer questions, address concerns and coordinate with meals, housekeeping, transportation and other supports to link residents with essential services.

The role emphasizes assessment, linkage to services, and advocacy within HUD guidelines, while collaborating with the Associate Director of Resident Services.

Qualifications

• Highschool Diploma or GED required.
• Bachelor's degree in social work, psychology or related field strongly preferred.
• Professional Service Coordinator designation preferred.
• Experience in an affordable housing environment preferred.
• Demonstrated ability to advocate, organize, problem-solve and deliver results for residents.

Responsibilities

• Contribute to caring and positive working environments for all employees and residents.
• Build rapport with residents while maintaining professional boundaries to help them age in place successfully.
• Identify resident needs and link services to promote independence and safety.
• Identify and facilitate resident activities and support groups each month.
• Assist residents with acquiring in-home support services where appropriate.
• Advocate for residents within HUD and regulatory requirements while protecting privacy.
• Model courteous, respectful communication and educate peers about resident needs.
• Other duties as assigned.
