Job summary

CCH seeks a Social Services Coordinator at Providence Senior Housing in San Francisco, CA to help senior residents maintain independence and dignity. You will answer questions, address concerns, and connect residents to meals, housekeeping, transportation and other essential supports while upholding privacy and respectful communication.

Reporting to the Director of Resident Services, you will conduct assessments, coordinate activities, and advocate for residents within HUD rules, while promoting

Qualifications

• Advanced interpersonal and communication skills (written and oral).
• Strong leadership and mentoring abilities.
• Analytical and problem-solving skills.
• Mediation and conflict resolution skills.
• Excellent organizational skills, detail orientation and time management.
• Proficient with Microsoft Office 365 and quick learning of other systems.
• Experience in affordable housing environment preferred.

Responsibilities

• Contribute to caring and positive working and living environments for all employees and residents.
• Build rapport with residents while maintaining professional boundaries.
• Support resident autonomy and dignity through service linkages.
• Identify resident needs and ensure contact for assessment/reassessment in last 12 months (≥95%).
• Identify, develop and facilitate/attend 90% of resident activities per month.
• Facilitate or arrange resident support groups and family communications as needed.
• Advocate for residents within HUD and regulatory requirements while protecting privacy.
