Qualifications

• Bachelor’s degree in social work, public health, psychology, or related field.
• 2+ years of experience in housing, social services, or human services.
• Bilingual in English and Spanish preferred.
• Knowledge of HUD regulations a plus.
• Experience with grants management a plus.
• Proficiency with MS Excel, PowerPoint, Word, Outlook.

Responsibilities

• Develop strategic partnerships with local service providers to bring on-site programs or refer to off-site programs across education, health, workforce development, financial stability, and asset building.
• Provide non-clinical case management and referrals to all residents, ensuring access to a full range of social services.
• Initiate strategic relationships with other local service providers and stakeholders.
• Collaborate with Property Management, family/eldercare providers, behavioral health, and medical professionals while maintaining confidentiality.
• Build relationships with residents to understand needs and connect with local resources.
• Conduct welcome orientation for new residents.
• Create outreach and communication with residents via newsletters, flyers, calls, and home visits.
• Document resident demographics, issues, requests, statistics, incidents, interactions, and outcomes.
