Qualifications

• Bachelor's degree in social work or related field preferred.
• Two or more years of experience delivering social services to elderly and/or family populations preferred.
• Working knowledge of supportive services and local community resources.
• Demonstrated ability to advocate, organize, problem-solve, and deliver measurable results for residents.

Responsibilities

• Advocate for residents and organize services to deliver results.
• Coordinate programs and activities for residents.
• Liaise with community agencies and service providers to secure affordable resources.
• Engage residents to identify needs and refer to community services.
• Develop a Resource Directory of local service providers.
• Sponsor educational events on health care, agency support, life skills and referrals.
• Provide quality customer service to residents and address concerns.
• Assist residents in accessing community services like housekeeping, meals, transportation, and daily support.
• Monitor feedback from residents and families and relay concerns.
• Manage relationships internally and externally with professional communication.
