Job summary

Marymount Hospital, part of the Cleveland Clinic, is seeking an RN Case Manager to collaborate with inter-professional teams and coordinate transitional care. This part-time, day-shift role emphasizes patient advocacy, holistic care, and safe handoffs across care settings.

Ideal candidates have at least 3 years of RN clinical experience, a BSN preferred, an active Ohio RN license, and BLS. Experience in utilization review, quality, or care management is preferred.

Qualifications

• Graduate from an accredited school of professional nursing.
• Current Ohio RN license and BLS certification required.
• BSN preferred; involvement in certification programs encouraged.

Responsibilities

• Ensure compliance with standards of accrediting bodies and state mandates.
• Promote interprofessional synchronization throughout the acute episode.
• Complete effective handoffs to the next level of care or community.
• Proactively assess and mitigate risks to patients’ transitions of care.
• Work with patients and care partners to develop individualized transitional care plans.
• Participate and lead in patient rounding activities, huddles, and committee meetings.

Education

Graduate from an accredited school of professional nursing.

BSN preferred.
