Job summary

Cleveland Clinic Marymount Hospital is seeking an RN Case Manager to coordinate transitional care across the care continuum. This part-time role supports effective handoffs and patient advocacy within a faith-based hospital setting.

Qualified candidates will be an RN with current state licensure and BLS certification; BSN is preferred. The schedule runs 8:00am–4:30pm with alternating 2/3 weeks, including a weekend every 6th and one annual holiday.

Qualifications

• Graduate from an accredited school of professional nursing.
• Current RN licensure in the state.
• BLS certification from AHA or Red Cross.

Responsibilities

• Promotes inter-professional synchronization throughout the acute episode and ensures effective handoff to the next level of care.
• Proactively assess barriers to transition and mitigate risk.
• Develop a comprehensive, individualized transitional care plan with the patient and care partners.
