Job summary

Cleveland Clinic Health System is seeking an RN Case Manager at its Main Campus in Cleveland. You will collaborate with inter-professional teams to coordinate transitions of care and ensure patient advocacy, holistic assessment and personalized care planning.

The role requires RN licensure in the state, BLS, and several years of acute care experience. You will participate in rounding, huddles, and committee work, supporting high-quality patient outcomes in a world-class health system.

Qualifications

• Graduate from an accredited school of Nursing and current RN licensure.
• BLS Certification (AHA or Red Cross) required.
• Bachelor’s degree in Nursing (BSN) preferred for some roles.
• Threeyears of recent RN experience preferred for advanced practice).

Responsibilities

• Collaborate with inter-professional teams across the continuum of care to facilitate and ensure effective transitional care coordination.
• Complete effective handoffs to the next level of care or community to ensure continuity of care.
• Proactively assess and mitigate risks posed by barriers to patients’ transitions of care.
• Develop a comprehensive, individualized transitional care plan with patients and care partners.
• Participate and lead patient rounding activities, huddles, and committee meetings.
